RN Telehealth Triage
Location: Minneapolis Minnesota

Humera is looking for a RN Telehealth Triage Nurse for a role at a large medical center and hospital located in Minneapolis. The ideal candidate would need to be able to work a flexible schedule.
- Consulting with patients or their caregivers over the phone, to assess the severity of symptoms and refer to the appropriate setting or level of care
- Alleviating suffering through nursing interventions and prescribed treatments
- Partnering with the patient, their family and significant others
- Documenting interventions and an individualized plan of care
- Educating the patient and thei! r caregivers on their disease process
- Assuring contin! uity of care Educating the patient and caregivers in prevention care which promotes health and well-being
- Current RN Licensure in Minnesota
- Compassionate, caring and customer service oriented
- Current BCLS certification
- Minimum of 2 years nursing experience in clinical nursing
- One + year of triage preferred
- Typing skills, 45 w.p.m. and good computer skills
- EPIC experience preferred
- Experience in Family Medicine including: Pediatrics, OBGYN, Geriatrics, etc preferred
- Evaluate and assess patient health needs, relative to acuity level
- Advise patients about health maintenance guidelines
- Prioritize the urgency and action needed to address the patient's request or problem
- Refer patients for additional health services based on their needs
- Recognize medical emergencies and initiate immediate action
The schedule for thi! s position is: 9:30pm-8:00am e/o Friday & Sat & Sun. 11:00am-9:30pm e/Wed. and 11:00am-9:30pm on the Friday's they are not working nights.
