Humera is looking for a RN Telehealth Triage Nurse for a role at a large medical center and hospital located in Minneapolis. The ideal candidate would need to be able to work a flexible schedule.
Consulting with patients or their caregivers over the phone, to assess the severity of symptoms and refer to the appropriate setting or level of care Alleviating suffering through nursing int! erventions and prescribed treatments
Partnering with the patient, their family and significant others
Documenting interventions and an individualized plan of care
Educating the patient and their caregivers on their disease process
Assuring continuity of care Educating the patient and caregivers in prevention care which promotes health and well-being
Current RN Licensure in Minnesota
Compassionate, caring and customer service oriented
Current BCLS certification
Minimum of 2 years nursing experience in clinical nursing
One + year of triage preferred
Typing skills, 45 w.p.m. and good computer skills
EPIC experience preferred Experience in Family Medicine including: Pediatrics, OBGYN, Geriatrics, etc preferred
Required Skill Sets
Evaluate and assess patient health needs, relative to acuity level Advise patients about health maintenance guidelines
Prioritize the urgency and action needed to address the patien! t's request or problem
Refer patients for additional healt! h services based on their needs
Recognize medical emergencies and initiate immediate action
The schedule for this position is: 9:30pm-8:00am e/o Friday & Sat & Sun. 11:00am-9:30pm e/Wed. and 11:00am-9:30pm on the Friday's they are not working nights.
