Fairview Clinics - Rosemount is seeking a Physician Assistant or Nurse Practitione! r to join a seven-practitioner practice. Provide outpatient care in an evolving, inter-disciplinary, primary care setting. Practitioners see about 20-22 patients per day.
Practice Information
On site services include: Family Medicine with obstetrics, Internal Medicine, Pediatrics, Podiatry, and Medication Therapy Management. Diagnostic services include lab and x-ray.
Practitioner expectation is 36 patient contact hours per week. Clinic hours are: Mon, Wed-Fri, 8 a.m. to 5 p.m. and Tue, 7 a.m. to 7 p.m.
The practitioners use a team approach in managing clinical care called Care Model Innovation This care model is used in providing care for their patients which actively looks at and changes clinical processes to maximize efficiency, resources, and clinical care. The site is has developed care packages and is a certified health care home.
Providers admit their patients to the hospitalist service at Fairview Ridges Hospital in! Burnsville.
