Detailed specification about this position opportunity kindly read the description below. At Fairview Clinics - Hiawatha, we are seeking a Physician Assistant or Nurse ! Practitioner to join an eight-practitioner practice. Be part of an outpatient practice with the opportunity to perform procedures. Clinic services include: preventive medicine, patient education, physical exams, treatment for minor injuries and care for acute and chronic illnesses. Practitioners see an average of 20-25 patients per day and enjoy a diverse, community based practice. Previous practice experience in a primary care setting required.
Practice Information
We are dedicated to providing comprehensive, high quality medical care for the entire family through all stages of life.
Onsite services include sports medicine, Ob/Gyn, podiatry, cardiology, nephrology and pharmacy and diagnostic services including lab, mammography and X-ray.
The practitioners work an expectation of 36 patient contact hours per week. Clinic hours are: Mon. and Thurs. 7 a.m. to 7 p.m. and Tues., Wed., and Fri. 7 a.m. to 5 p.m.
We have a fully integrat! ed Electronic Medical Record (EMR) and every provider uses the! EMR.
Providers admit adult patients to the Fairview hospitalist service at the University of Minnesota Medcial Center, Fairview.
The practitioners use a team approach in managing clinical care. The site is using a new model, Care Model Innovation, in providing care for their patients which actively looks at and changes clinical processes to maximize efficiency, resources, and clinical care. The site is also has developed care packages and became a certified health care home.
