At Fairview Clinics - Brooklyn Park we are seeking a Physician Assistant or Nu! rse Practitoner with previous primary care experience to join our multi-specialty practice. This is a thriving outpatient practice with a diverse patient base. Our group is very procedure-oriented and encourages providers to utilize their skills and special interests. The Physician Assistant or Nurse Practitioner will have his/her own patient panel and an Internal Medicine emphasis on his/her practice. We are seeking someone who is very comfortable and excited by innovation and changing the model of care. Previous practice experience is required. Ideal candidate will have experience with and an interest in working with geriatric patients.

Fairview Clinics - Brooklyn Park is dedicated to providing comprehensive, high quality medical care to the entire family through all stages of life.
The primary care providers work collaboratively with 20+ on-site practitioners in these specialties: family medicine, internal medicine/pediatri! cs, Ob/Gyn, internal medicine, general and orthopedic surgery,! otolaryngology, occupational medicine and dermatology. Many other subspecialists are available through extensive outreach programs. Urgent care services available on-site.
Onsite services include rehabilitation services and diagnostic services including lab, X-ray, optical dispensary and pharmacy.
Practitioners work an expectation of 36 contact hours and a five day work week.. Clinic hours are Monday-Thursday 7 am - 7 pm and Friday 7 am - 5 pm. Urgent care is available on-site evenings and weekends.
The practitioners use a team approach, Care Model Innovation, in managing clinical care. Care Model Innovation actively looks at and changes clinical processes to maximize efficiency, resources, and clinical care. The site is incorporating care packages and will become a certified health care home.
