At Fairview Clinics - Bass Lake we are seeking a board certified/board eligible Physicians Assistant or Nurse Practitioner to join a group of three physicians. Opportunity to perform procedures is based on your interests. This highly regarded and successful practice offers a full array of on-site services to meet the needs of the community. Previous experience in a primary care managing a patient panel is required.
Practice Information
We are dedicated to providing comprehensive, high quality medical care for the entire family through all stages of life.
Onsite services include lab, X-ray and OB care.
The practitioners work a 5-day workweek with an expectation of 35 patient contact hours per week. Clinic hours are: Monday - Tuesday, 7:30 a.m. to 7 p.m.and Wednesday - Friday, 7:30 a.m. to 5 p.m.
We have a fully integrated Electronic Medical Record (EMR) and every provider uses the EMR.
Practitioners admit their patients to the hospitalist service at Maple Grove Hospital.
The practitioners use a team approach in managing clinical care. The site is using a new model, Care Model Innovation, in providing care for their patients which actively looks at and changes clinical processes to maximize efficiency, resources, and clinical care. The site is also has developed care packages and became a certified health care home.
